Please have the following information available when calling to schedule an exam:
- Request (script) for a specific procedure or test signed by your doctor or referring physician
- Referring physicians contact information
- Insurance information
- Medical record number (orange hospital plate)
First-time patients will need to get a Johns Hopkins Medical record number. The following information is needed for that purpose:
- Patient’s first and last name
- Date of birth
- Race and gender
- Patient’s address and phone number
- Father’s name and mother’s maiden name
