Frequently Asked Questions from our patients
Submitting Outside Images: All imaging studies that are to be stored in the JHM Image Archive require an official Johns Hopkins interpretation – see policy below. If outside images were interpretated by a Hopkins radiologist from an affiliated site, then image consultation is not required to import images into the archive. Policy for Outside Image Consultations: - The requesting physician is responsible for notifying his/her patient that a minimal fee will be charged for requests for outside consultations and storage of images on the archive.
- Referring physicians are required to complete a requisition for an outside image consultation. (Requisition form available by clicking here)
- Each study date requires a separate Outside Image Consultation requisition.
- Information required on the requisition include: patient’s name, JHH medical record number, birth date, and clinical history.
- Whenever possible, submit the outside image study report(s) with the CD and/or films.
- An accredited physician in the Department of Radiology will review and formally interpret the outside study.
- Patients need to pick up outside films or CDs within two weeks after images are imported into the archive. Unclaimed films and CDs will be discarded after two weeks.
What happens if Radiology cannot import your CD into the imaging archieve? - Radiology estimates that about 15 percent of CDs cannot be imported into UltraVisual. When this occurs, the order form will be viewable in UltraVisual with the imprint *VIEW ONLY* on the form.
- In the event the CD cannot be imported, you will be notified to pick-up the CD immediately.
- CSC is not responsible for storage of unclaimed CDs or films after two weeks from day of submission of the items.
